To gain access to a PBHMD open space, please complete the 'Usage of Open Space Contract' below and submit. A $25.00 fee is required and must be paid prior to access. We accept payment in office via check or card. You can also pay online at www.pbhmd.org or call us at (719)495-8188 to pay by phone once contract has been processed and submitted.
