Customer Activation/Deactivation Request

NOTE: If you are purchasing a home, your title company should forward a 'Property Utilities Settlement Form.'  Please contact your title company to ensure this has been sent.

 

TITLE COMPANIES: Please send us the completed 'Utility Settlement Request' to start the process of activating service for the new owner.

 

OWNERS WITH RENTAL PROPERTIES: Please fill out 'New Owner Contract.' Your tenants will complete their form.  

RENTERS: Please complete  and return the 'New Tenant Contract' along with your 'Lease Agreement.' The dates of requested service must match the dates on the Lease Agreement.

PROPERTY MANAGERS: Please send over the Property Management Agreement and any other documents necessary depending on request.
